Everything you want to know about the Bloom Metabolic Program, Direct Primary Care, insurance, pricing, and what to expect.

The Bloom Metabolic Program

What is the Bloom Metabolic Program?
A comprehensive, physician-led approach to weight and metabolic health. The program includes a thorough metabolic intake, close follow-ups with Dr. Katya every 4 to 6 weeks, comprehensive labs and body composition scans throughout, personalized nutrition and movement planning with support from a partnering registered dietitian, full treatment exploration including medications when appropriate, and a real long-term maintenance plan. See the full program details.
Who is the program for?
Adults seeking comprehensive metabolic care. Common reasons people enroll include weight loss, insulin resistance or prediabetes, metabolic syndrome, cardiovascular risk reduction, weight regain after past treatments or surgery, and long-term maintenance after medication-assisted weight loss. Whether you're starting from scratch or coming off another program, we tailor the approach to where you are.
Are medications required to join the program?
No. Medications are one tool in the toolkit, not the focus of the program. Some patients do well with lifestyle-focused care alone; others benefit from GLP-1s, oral medications, or other treatments. Your plan is built around what your body needs, what you'll respond to, and what's safe long-term.
How long is the program?
The program is designed around a 12-month treatment arc with a 6-month minimum enrollment. We use that time to evaluate, treat, and reassess at the cadence real metabolic change requires.
What's included in the monthly program fee?
Your monthly fee covers a comprehensive metabolic intake, regular physician follow-ups every 4 to 6 weeks, comprehensive labs at intake and at two additional time points, body composition scans (SECA) three or more times throughout the program, personalized nutrition and movement planning, direct messaging with Dr. Katya, treatment exploration including medication management, and a long-term maintenance plan. Medications themselves are billed separately. See the full list.
How is this different from online weight loss programs?
Most online weight loss programs are essentially medication businesses: a short virtual visit, a prescription, a monthly fee, and a different provider next time. Bloom is built around real medicine. In-person physician care, deep evaluation, included labs and body composition scans, the same physician at every visit, and a real maintenance plan. Medication is one tool we may use, but it isn't the business model.
What happens after I complete the 12-month program?
You have options based on where you are. If you still have active metabolic or weight goals and aren't yet ready for maintenance, you can continue the metabolic program month-to-month at the same monthly pricing. If you've reached the maintenance phase, you can transition to Direct Primary Care, where Dr. Katya continues as your primary physician, or step down to twice-yearly maintenance visits if you don't need full DPC membership. We work with you on the path that fits.

About Direct Primary Care

Is Direct Primary Care for me?
If you value having a real relationship with your doctor, getting same or next-day access, longer appointments, and transparent pricing, DPC may be a great fit. It is ideal for anyone who wants proactive, personalized healthcare without the red tape of insurance-driven medicine.
Is DPC the same as insurance?
No. DPC is not insurance. It's a membership that covers your primary care. We recommend pairing DPC with a catastrophic or high-deductible health plan for hospitalizations, emergencies, and specialist care. Many members find this combination saves them significant money overall.
What makes Bloom different from other DPC practices?
Beyond comprehensive primary care, Bloom specializes in metabolic health. Dr. Katya is board-certified in obesity medicine and takes a root-cause approach to conditions like insulin resistance, diabetes, and obesity, combining evidence-based medicine with personalized lifestyle guidance.

Insurance & Cost

What does Bloom cost?
The Bloom Metabolic Program is $250/month with a 6-month minimum, or $300/month bundled with Direct Primary Care (Bloom Complete). Direct Primary Care will be $125/month per adult when it opens later in 2026, with couples and family rates available. Non-member visits are $200 each. Comprehensive labs and body composition scans are included in the metabolic program; medications are billed separately at near-cost or insurance-covered pricing. See the full pricing breakdown.
Do you accept insurance for care at Bloom?
No. Bloom is a Direct Primary Care practice, so we don't bill insurance for our membership or visits. This keeps care affordable, predictable, and focused on you instead of paperwork. You can still use your insurance for hospitalizations, specialist referrals, imaging, lab work outside the metabolic program, and prescriptions.
Can I use my HSA or FSA?
In most cases, yes. Direct Primary Care fees and medical services at Bloom are typically eligible for HSA and FSA reimbursement. We recommend confirming eligibility with your specific plan administrator, as rules can vary by employer. We can provide itemized receipts for your records.
Will my insurance cover obesity medications?
Many insurance plans cover at least some weight management medications, but coverage varies widely by plan. If you have insurance, we help you pursue coverage including completing prior authorization paperwork and handling appeals when needed. For medications your insurance won't cover, we offer near-cost direct pricing through Bloom that's often comparable to or better than typical cash prices at retail pharmacies. See medication options and pricing.
Do you accept Medicare or Medicaid?
At this time, Bloom is unable to accept patients enrolled in Medicare or Medicaid. We hope to expand access in the future. If you're enrolled in one of these programs, you can join our waitlist for updates.
Will my insurance cover lab work and imaging?
Comprehensive metabolic labs and body composition scans are already included in the Bloom Metabolic Program fee, so there's no separate billing for those during the program. For labs, imaging, or testing outside the metabolic program, you have options: we can order them through your insurance via your preferred lab or imaging center, or use cash-pay pricing through Bloom, which is frequently significantly discounted compared to larger clinics.
What about specialists or hospitalization?
For specialist care, imaging, hospitalization, or emergency services, you use your health insurance. Dr. Katya helps coordinate referrals and follows up with you after specialist visits or hospital stays.

Visits & Access

How do I reach Dr. Katya between visits?
The preferred way to reach Dr. Katya is through the patient portal, where messages are tracked alongside your chart. Calling the clinic is also an option. Either way, you're reaching Dr. Katya directly, with no phone trees and no waiting for a callback from a nurse. For non-urgent questions, expect a response within one business day.
What happens if I need to be hospitalized?
For hospitalizations and emergencies, you'd use your health insurance or catastrophic plan. Dr. Katya will be available to help you navigate your care and follow up with you after any hospital stay.
Do you offer telehealth visits?
Yes. Members have access to both in-person and virtual visits. Telehealth is available for many types of visits, making it easy to connect with Dr. Katya from wherever you are.
Can I see Dr. Katya without a membership?
Yes. Non-member visits are available for $200 per visit. This is a good option for one-time concerns, second opinions, or trying Bloom before committing to a membership or program. Learn more about non-member visits.

Getting Started

When does Bloom open?
Bloom is now seeing patients. Complimentary Meet & Greets are open for booking, and Dr. Katya begins seeing Bloom Metabolic Program patients on May 18, 2026. Direct Primary Care membership enrollment will follow later in 2026. Join the Bloom List to be notified when DPC opens.
What is a Meet & Greet?

A short, complimentary visit (virtual via Google Meet, or in person) where we can get to know each other, talk through your goals, review program options, and answer your questions. It's not a medical visit, so no diagnosis or treatment is provided.
